Surgery is rarely used as a treatment for IC/BPS and is a final treatment option, reserved for patients who have a small, fibrotic bladder and/or the presence of Hunner's lesions that have not responded to treatment.
Patients who are at a much higher risk of failure with surgery are those who have a large bladder capacity, who don't respond to an anesthetic cocktail (which verifies that their pain is coming from the bladder), who have pelvic floor tension and/or pain beyond the bladder (widespread pain).
Pain can persist even after bladder removal because, for most IC/BPS patients, it is driven by nerves and muscles rather than the bladder wall.
There are three types of surgery available:
(1) Augmentation Cystoplasty - which expands bladder capacity using a section of bowel.
(2) Urinary diversion - which bypasses the bladder
(3) Cystectomy - which removes the bladder
The surgeon performing the surgery should have years of experience with IC/BPS and be dedicated to your long term care and recovery.
Out of sheer frustration and pain, many patients consider bladder removal as their last chance at treatment. Thankfully, now that we are using phenotyping to understand the underlying cause of pain, more and more patients are finding their correct and most effective treatment.
