Transgender post-op pelvic pain, is pain that persists after a gender affirmation or bottom surgery, that significantly changes the anatomy of the pelvic floor. The most common causes of chronic pelvic pain post-surgery can be secondary to adhesions or scar tissue, which can affect blood flow and cause nerve pain. There can be pre-existing conditions that can contribute to a higher risk of post-op pain. In trans patients who are assigned female at birth, they can have underlying gynecological disorders such as endometriosis or fibroids, which can put them at a higher risk for that pain.
Transgender post-op pelvic pain is diagnosed with a thorough history discussing urinary, bowel, sexual symptoms and pain, along with a thorough physical examination. The treatment plan consists of physical therapy, medication management, techniques such as diaphragmatic breathing, maybe starting to include yoga into your exercise routine. We can also do peripheral nerve blocks and trigger point injections to the pelvic floor muscles to help the neuromuscular cause of post-op pelvic pain. Pelvic rehabilitation medicine is a gender inclusive practice.
