A few other things to keep in mind:
The greater the difference in orbit between the two vessels, the easier it will be to set up a rendezvous, but the more delta-V you'll have to use to match the orbit once you get there.
Also, turn off your RCS thrusters and/or SAS if you're not using them, since having both on can mess up your path.
Sometimes the intersection nodes spaz out once you get them to line up. I don't know how to solve this; you just have to muscle through it.
