Boracay Requirements Update February 2022
All tourists and travelers shall secure a “Quick Response” (QR) Code via the Online Health Declaration Card (OHDC) that can be accessed at https://www.touristboracy.com/ or https://aklan.gov.ph.

Philippine government-issued ID with Aklan address
Travel Details
For below 12 years old (accomplish the certificate of undertaking travelling w/minors template at https://aklan.gov.ph/ )
Proof of Vaccination
Fully Vaccinated Individuals (any of the following)
VaxCertPH Covid-19 Vaccination Certificate w/ full vaccination status
Locally issued vaccination card/ID w/full vaccination status and verifiable QR code or with contact details of the vaccination center
World Health Organization International Certificates or Vaccination Card/ID/Certificate issued abroad
Unvaccinated/Partially Vaccinated Individuals
Negative RT-PCR result certificate taken from any DOH-accredited laboratories within 72 hours to date of travel

Send the following documents to [email protected], using the email subject OHDC-Family Name, First Name- Date of Travel (MM-DD-YYYY)
Notice of Coordination/Acceptance from municipality of destination issued by the Office of the Mayor

Travel Details (only Kalibo International Airport can be used by those travelling via air)
TOURISTS
Proof of Identification with Philippine Residency/ Passport for foreigners/OFWs/OFs
A copy of confirmed booking slip/form from a DOT - Accredited Boracay Island accommodation/establishment
Round Trip flight/travel details (only Caticlan Airport can be used by those travelling via air)
Proof of Vaccination Status
Fully Vaccinated Individuals (any of the following)
VaxCertPH Covid-19 Vaccination Certificate w/ full vaccination status
Locally issued vaccination card/ID w/full vaccination status and verifiable QR code or with contact details of the vaccination center
World Health Organization International Certificates or Vaccination Card/ID/Certificate issued abroad
Unvaccinated/Partially Vaccinated Individuals
