How did Joanna Lannister know that her twins were incestuous
There are three reasons I can find for Joanna not to tell Tywin, and it was most likely a mix of them.
The first reason is that Jaime and Cersei were very young children. I think they weren't even 10 years of age, and when you are that young, people don't seek to demonize you but rather to educate you. She probably figured that they didn't know the meaning of what they were doing, so she decided to just educate the two.

The second reason is that she didn’ t know she would have soon died. Had she survived Tyrion's birth, it's likely that the twincest would have been stopped.
The third one is that Tywin isn't exactly reasonable at all times. He destroyed an entire castle just to make an example for other rebellious lords, so it's possible that if he had known he would have tried to put an end to it in a more drastic way.
