You will need minimum of 2 playthroughs - on first one aim to restore vestiges before successor boss fights and before final boss restore all Io Eos vestiges. On your second do not pick up any vestiges, meaning don't restore any successor, which will give you different set of blood codes.

3:44 - Heimdal Blood Code - Aquired by speaking to Jack at the Home Base after Successor of the Throat boss in Crown of Sand
3:58 - Hephaestus Blood Code - Restore Hephaestus Vestiges and talk to Murasame to get her Blood Code (TUTUT)
4:27 - Hermes Blood Code - Aquired by speaking to Davis at the Home Base after Insatiable Despot boss in Dried-up Trenches. Posibly you may need to restore some Hermes Vestige to trigger it

5:16 - Isis Blood Code - [MISSABLE] You must collect all Isis Vestiges in Cathedral of the Sacred Blood and restore them to the Io clone in front of the boss room before defeating the boss Successor of the Ribcage. Afterwards restore Aurora (TUTUT)
