Dr. Obzejta explains what a hypertonic pelvic floor is and the symptoms associated with a pelvic floor that is in spasm.
Dr. Natalia Obzejta, MD is an expert in women’s and men’s health and sexual health, who believes in taking an integrative, functional, and holistic approach to treatment and the importance of preventative maintenance and medicine.
At Pelvic Rehabilitation Medicine™, we believe that the millions of women and men who suffer from pelvic pain should have access to care that provides relief. We don’t believe “a glass of wine will fix it” or that “it’s all in your head”. We will never abandon you – and from leadership to the front office to every pelvic pain specialist you meet, so much of our team has been in your shoes and believes the pelvic pain patient deserves better. This belief is what unifies us all at PRM.
Since 2017, PRM has provided an innovative solution to a health crisis that affects 15% of women and 10% of men. Persistent pelvic pain is often overlooked, untreated, and misguided. At PRM, our mission is to decrease the time patients are suffering from pelvic pain symptoms.
We offer a proprietary, simple, office-based procedure to treat the symptoms of chronic pelvic pain. The PRM Protocol™ consists of a series of pelvic nerve and muscle treatments to directly target inflammation in the pelvis and nerve pain. Depending on the condition causing your pelvic pain, other modalities may be needed in addition to our treatment.

So we see a lot of patients here in our clinic who have what we call hypertonic pelvic floors. It's a fancy word that basically just means, your pelvic floor is in a spasm. So when you think of a spasm, if you ever had like a neck spasm or like a spasm in your back, that's exactly the same thing that's going on, right?
So typically the pelvic floor is supposed to relax and contract nicely, right? So during our physical exam with our patients, we'll ask them to do a pelvic lift, kind of like if you're holding in urine or the "kegel" we can feel that nice lift so the muscles are contracting and then they will relax appropriately.
Most of our patients that come through the door have hypertonic pelvic floor, which means the pelvic floor is in that spasm. So the muscles cannot contract and they cannot relax appropriately, which can lead to a lot of symptoms such as constipation, difficulty having a bowel movement, and Sharp rectal pains, urinary urgency, hesitancy, delayed urinary stream, that sensation of incomplete bladder emptying, painful intercourse, chronic low back pain, tail pain, so it can cause a lot of stuff that kind of, you know, sounds and feels like It's something else.
We don't typically think of the pelvic floor of causing all these symptoms, but it can. It's super important, right? And we all have a pelvic floor. So super important, you know, if you're having any of these symptoms and you can't figure it out, come see your local pelvic pain specialist and, you know, we'll take a look at your pelvic floor and see if we can help you.
