A Swedish study published in JAMA (July 2024) found that a blood test measuring amyloid-β and phosphorylated tau levels identified Alzheimer's disease (AD) with about 90% sensitivity and specificity. The study involved around 1,200 patients with cognitive symptoms, showing that the blood test's accuracy (91%) surpassed clinical diagnosis by primary care physicians or dementia specialists. However, the positive predictive value of the test decreases in populations with lower AD prevalence. While not yet FDA-approved, these tests may soon be available, raising questions about their appropriate use, particularly in primary care settings, given the potential for misdiagnosis and the complex nature of cognitive symptoms.
