A child with leukemia brings a message to humanity—No one can take charge of other people’s lives except God ,each of us is just a part of the puzzle,Without love, humanity would have no hope at all .
The autonomy in medical decision-making has always been a controversial topic. In today’s healthcare system, however, faced with complex medical conditions and uncertain disease processes, this autonomy is often challenged.
Our experiences over the past decade have deeply taught me the value of patient and family autonomy. My daughter, Bubu, has experienced four relapses of leukemia over these years. The first treatment lasted for two and a half years, with a remission of five months before relapse. The second treatment led to a major medical accident due to aggressive chemotherapy, leaving the child in a state of brain death at home awaiting death. If we hadn’t persevered, our child might have lost her life long ago. On the advice of doctors, the third time we decided to try the latest Car-T therapy in the world.
Car-T therapy has been hailed as a new hope for leukemia treatment, by modifying the patient’s own T cells to more effectively attack cancer cells. However, this treatment is not without significant side effects. Even so, Bubu experienced extensive rashes and bleeding from multiple orifices, yet she managed to pull through. After undergoing Car-T therapy, her condition improved somewhat, but only for a short 90 days before relapsing again.
These 90 days meant everything to us. This time not only bought us time for successful hematopoietic stem cell transplantation but also gave us hope. However, this time of hope was quickly shattered by repeated relapses. This time, we learned not to dare to rejoice loudly, for fear of waking up the sorrow next door. Each improvement in her condition was accompanied by anxiety about another relapse. And this time, Bubu’s lungs were infected to the extreme, with extensive necrosis appearing on her body. The hospital doctors gave up treatment for a hundred reasons and planned to remove her oxygen.
However, before I reached the ward, I posted a “Do Not Enter” sign for the doctors and sent two doctors out successively. I cried, having gone through so much, I couldn’t fully trust them, I knew how serious my daughter’s illness was, I didn’t need five or six doctors coming into our room every day telling me how bad my daughter was! But I had no other way, still according to the doctor’s instructions, I informed our relatives and friends to come for the final farewell. My last call was to Dr. Jiang Mingyao, I saw his WeChat Moments, knew he was attending a meeting in Washington, hesitated several times in my heart whether to tell him this terrible news, because I knew how busy he was. And every time I wanted to give up this call, it seemed that there was another force urging me to call him, call him.
Finally, I gathered the courage to dial a string of numbers and informed him of Bubu’s condition. I didn’t want to disturb him too much, I just wanted to ask if he could attend Bubu’s funeral. Dr. Jiang Mingyao had saved Bubu from the clutches of death before, and I felt that he needed to know about my daughter’s condition before her end. Dr. Jiang Mingyao inquired about Bubu’s current condition while comforting me to stay calm. He suggested that I should immediately go home and make a special herbal concoction for two hours, striving to give Bubu the last hope and treatment. At the same time, he called on all members of the medical team to hold a family meeting on Monday to discuss patient autonomy issues.
In the end, Bubu miraculously survived again. Although the disease brought us endless pain and struggle, our perseverance eventually carried us through the ordeal.
This experience has deeply taught me how important autonomy in medical decision-making is. When facing medical choices, we should not only listen to the doctor’s advice but also maintain control over the treatment process. It is this autonomy that has given us hope and enabled us to pursue every possibility of life.
