So, after windows update some cards just did not want to work...
The soultion is actually quite simple.
1. Reinstall / patch first blockchain drivers (if for some reason m$ decided to replace your drives as well)

2. Change the swap file size to 32GB (if that does not work then to 40... etc... untill it starts to work propertly)
3. Profit??? :)
I assume this is not the issue if you have 16GB of ram (or maybe 12) however, with 8GB it does seems to be a problem.
*** IMPORTANT: You need to have the latest version of miner!!!
*** if you have nVidia cards and you still have issues with dag creation try adding: "-eres 0" to your batch file
