Dr. Melanie Howell discusses chronic prostatitis and how it can be treated.

At Pelvic Rehabilitation Medicine, our pelvic pain specialists provide a functional, rehab approach to pelvic pain. When you visit one of our offices, you spend an hour with your doctor reviewing in detail your medical history and symptoms. Then, we perform an internal exam (no speculum) to evaluate your nerves and muscles. Together, we'll discuss an individual treatment plan that gets to the root cause of your pain and helps you to feel better. The best part: you can begin treatment the same day!

At PRM, our mission is to decrease the time patients are suffering from pelvic pain symptoms.

Chronic prostatitis is a condition that involves an inflamed prostate, and is often in association with a hypertonic pelvic floor, as well as some inflamed nerves, surrounding nerves within the pelvic floor.

Some of the symptoms that I'll often see with chronic abacterial prostatitis are urinary frequency, urinary burning, urinary hesitancy. And this happens usually because the fact that the prostate, as well as the ureter, are in close proximity and are coursing through the pelvic floor musculature. So, oftentimes that musculature will squeeze around the ureter or squeeze in close proximity to the prostate and cause inflammation and cause pain, and then you'll see some of these urinary symptoms alongside it.

In this type of treatment, I'm oftentimes working with a urologist side-by-side, so we can try to treat some of these symptoms. So, occasionally we'll prescribe medications that will address the urinary urgency and frequency, we'll prescribe pelvic floor physical therapy to try to relax some of the musculature that is causing some of the symptoms. Oftentimes we'll prescribe medications that will also relax the musculature through the use of suppositories. And then we'll also do our injection protocol to relax the musculature through trigger point injections, as well as decrease the inflammation of the nerves involved.
