How Bridge OBD Terminals Plug to get fault codes . Always check the basics first !
Transmission Limp Mode ( limp home mode, limp in mode, fail code etc. ), transmission stuck in one Gear, is when the computer senses an electrical reading outside the set parameters, the computer then sets vehicle into limp mode to save any or further damage that could be caused. Check Engine light may be on or flashing.
In a 4L60E transmission ( this one was a VS Holden Commodore / Statesman ) it is set to 3rd gear. This is the setup that is used if you pull the electrical plug right off the transmission. No electrical power is energizing any of the transmission solenoids. If you check your solenoid application chart, it will show that this is 3rd gear.

MIL (malfunction indicator light) on/flashing
CEL (Check Engine Light) on/flashing
System Fault Codes malfunctions fall into 2 Categories:
1. Hard Faults.
These are indicated by the MIL (malfunction indicator light) being illuminated; Whenever the Engine is Running.
2. Intermittent Faults.
These cause the MIL to be illuminated while the Fault is Present.
The MIL will go out approx. 10 Seconds after the Fault Disappears.
Faults of Either Type will cause a Fault Code to be stored in Memory.
Fault Codes will be displayed on the Check Engine Light.
All Codes will consist of Two Digits.
If No Fault Codes are stored in Memory;
A Code 12 will be displayed.
This consists of;
One Flash of the MIL
Followed by a 1.2 Second Pause
Two Flashes of the MIL
Followed by a Longer Pause.
The Code will be Repeated.
This Sequence will continue;
